Membership Agreement

About Gramatan Village

Gramatan Village, Inc. (“GV”) is a New York State nonprofit organization and a registered 501(c)(3) charitable organization. Our mission is to help older adults live comfortably, confidently, and independently in their own homes as they age.

Through trusted local partners and community resources, Gramatan Village offers access to social activities, educational programs, helpful services, volunteer support, and referrals designed to support an active and connected lifestyle.

Membership Terms

Your annual membership begins on the first day of the month in which your membership payment is received and continues for twelve (12) consecutive months.

Members who choose a monthly payment option agree to a recurring monthly credit card charge for a twelve-month membership period.

• Annual memberships are non-refundable after the first 30 days.
• Membership renewal notices will be sent prior to the expiration of your current term.
• Gramatan Village reserves the right to update membership fees with advance written notice.
• Membership is personal and non-transferable.

Services & Providers

Gramatan Village works to identify activities, programs, and service providers that meet our members’ needs and interests. Many services are offered through carefully selected third-party providers who may offer convenient scheduling and, in some cases, discounted pricing for members.

In most cases:

• Members contract directly with third-party providers.
• Members are billed directly by those providers for services received.
• Gramatan Village does not directly provide medical, home care, legal, or financial services.

Communication & Member Support

To better understand and support members’ needs, I authorize third-party providers to share limited non-medical information with Gramatan Village regarding the services I receive.

I also understand that Gramatan Village may contact my designated emergency contact or a family member if there are concerns regarding my health, safety, or well-being.

Release of Liability

As a member of Gramatan Village, I acknowledge and agree to the following:

1. 1 I understand that services arranged through Gramatan Village are provided by independent third-party providers.
2. 2 I release and discharge Gramatan Village, Inc., its officers, directors, employees, and volunteers from any responsibility or liability for services provided by third-party providers.
3. 3 I agree to hold harmless Gramatan Village from any claims, costs, expenses, or damages — including reasonable attorney’s fees — arising from services provided by third parties, including claims brought by me, on my behalf, or by my insurance carrier.
